In order to send a patient a questionnaire, they first must be registered for Arya's PHR. Please see our help article on inviting a patient to register their patient portal here.
Sending a Survey/Questionnaire to a Patient
Once a patient has their portal account enabled, in addition to being able to share Notes, Results, Forms and Prescriptions with them using the Share button from the respective tabs. You are now able to share questionnaires to them to complete
Let's try sharing an Intake Questionnaire with a patient.
1. Open the patient's chart and click on the "Portal" tab on the blue patient navigation bar
2. Click "New" to select from your clinic's available questionnaire templates.

3. Click "Select" to add the template to your patient's portal. This does NOT send to the patient immediately. You are able to preview the questionnaire content before sending off to the patient
4. When you are ready to send the questionnaire off, click on the questionnaire you wish to share. Then click "Share" to send a notification to the patient's email that we have on file.
5. All questionnaires have to be assigned to a clinical user before being shared with patients.
If you are a Clinical User: Arya will automatically assign the questionnaire to you. This assignment will be visible in the portal global tab when filtered by the clinical user.
If you are a Non-Clinical User: you will be prompted to select a clinical user from a dropdown menu. Once selected, the Share button will pop up, indicating readiness to share the questionnaire with the patient's portal.
6. Done! The patient can log into their patient portal and respond to the questionnaires. You will not get a notification that the patients has completed the questionnaire, their response will automatically appear in the portal tab.

Once a template is added to the portal tab, you can eFax, Print, or Delete the template at any time. This means you can share a blank template with another provider via eFax, or send off the patient response in the form of an eFax once the patient responds.
