When creating a questionnaire, you will need to be in the Survey Engine. You can navigate to the dropdown menu in the top right corner and select Admin. This will bring you to the admin panel where the Survey Engine can be located.

Once you are within the admin panel, you can select the "Survey Engine".

Once you're on the Survey Engine Page, Click the "New" button located on the left "Templates" panel to create a new survey template. You can add a title , select the clinic, and select if you would like this questionnaire to be private

Once you have done the above, you will also have the option to select "Generate Note"

When you are creating the questionnaire questions, you will now have a textbox "Show As:" that will allow you to manipulate how the questionnaire will be shown as a note.

A "Placeholders" side panel will be on the right side. When you have created the question and added any necessary selection options, you can utilize the placeholders within the "Show As" section. You can personalize how the questionnaire appears as a note. By selecting the placeholders on the right hand side, you can create how you would like the note to be organized.

If you are creating a Likert Scale question, a "Show As:" line will appear under each statement.

The Placeholders on the right side will populate within "Show As" as per the selection made.

Once you have completed the questionnaire and hit "Publish", you can then choose to send it to a patient for completion.

Go into the patient chart and select "Notes".

On the left hand side, the questionnaire that was completed by the patient that was created within the Survey Engine will appear.

You can select the questionnaire and it will appear with the completed questionnaire. How it is displayed is how it was inputted within the "Show As" section of the questionnaire.
