Duplicate patient charts can complicate record-keeping and may lead to errors in patient care. It's important to maintain accurate and organized patient records. This guide provides step-by-step instructions on how to merge duplicate patient charts effectively.
Navigate to your duplicate patient chart and from here, there are 2 options to go about this:
eFax - fax all documents to your clinic and assign to original patient chart
Saving PDFs - all documents are downloaded and re-uploaded to the original patient chart

The first thing you want to do is to collect documents associated with the duplicate patient chart. You can look through the following patient tabs and click on eFax :
Once all of the documents has been queued for faxing, you can fax it to your clinic's fax number. Once this has been received within your eFax inbox, you can re-assign to the correct patient.
π NOTE: When faxing them all together, they will be received as one fax. We recommend individually faxing the documents so they can be assigned as separate documents.
Save as PDF
You can save each document as a PDF to your computer, then navigate to the correct patient chart and upload them to the corresponding locations in their chart.
To save as a PDF, you can select the Print icon and change the Printer option to Save as PDF. This will allow you to save the document to your computer.
Once the documents have been saved as a PDF, you can navigate to the correct chart and upload the documents to the corresponding locations in their chart.
Removing the Duplicate Chart
Once one of the above two options is completed, you can navigate to the Patient List and delete the patient.
