How to Complete Patient Forms and eFax from the Patient's Chart

This article will provide you with a guide on how to fill and complete forms and eFax from the patient's chart directly.

Completing a Form

When you are in a patient's chart, you can navigate to the Forms tab. This will provide you with a list of Forms previously done for the patient. By selecting on a form in the list, will bring up the form for you to view or edit.

You can select New if you wish to create a new form for the patient. This will provide a list of forms that have been created in Arya. You can search and choose the form you wish to complete for the patient. Once you have found the form, you can hit Select.

Most forms will have the patient and physician information generated onto the form so you do not have to manually fill this in. Depending on the form, there will be text areas and checkboxes for you to complete for the form.

This allows for you to electronically fill and complete requisitions, referral forms, etc. rather than manually completing them by pen and paper.

You can select Upload, if you wish to upload a form directly from your computer. By doing this, the form will not have the same functionality as a form created in Arya.

eFaxing a Form

Once you have completed the form and have filled in the necessary fields, you can select eFax to queue the form for electronic faxing.

Once you have selected eFax, this form will be added to the Outbox in the eFax tab. You can navigate to the eFax tab in the patient's chart to find any queued documents for the patient.

The Outbox will be a list of documents that have yet to be eFaxed. You can input the recipient name or fax number you wish to fax the form to. You can also select Address Book, if you would like to search for the recipient. Once you have inputted who you wish to fax to, you can select either Send (keeps the document in the Outbox) or Send & Clear (to clear the document from the Outbox).

Soon you will be able to search forms by province, jurisdiction, or by form type when adding a new form.
