Inputting Patient Demographics

How to input patient demographics correctly into the patient chart, and any questions you may have about it!

In order to add, change, or remove certain parts of patient demographics, here is how you can do that.

Opening Patient Demographics

1. Search for the patient under Home, that you are looking for. If the patient doesn't exist, click on Add Patient on the right side, to add a new patient.

2. Click on that patient to open up their Profile.

3. Under Profile, click on demographics to open up the Patient Demographics. This is where you will find all the primary information about the patient such as address, date of birth, gender, emergency contacts, phone numbers, and more!

Here are some tips and useful information when inputting patient demographics:

1. When inputting Insurer, you have the option to pick if the province covers their health, if they have no insurer, or if they have a private insurance.

2. For Health Number, ensure you put in the correct 10-digit health number for it to register into the system.

3. For Gender, there are several dropdown options you can pick from!

4. For Patient Status, you can update or modify this when a patient leaves or passes away, with the dropdown options provided.

5. You can add multiple referring physicians if needed! Just select the + button to add more.

6. Patients can opt in to receiving healthcare information via the patient portal, once you enter in their email information.

7. Ensure you select which phone number that was inputted is the primary one for the patient. This will help when needing to reach out to the patient for any information.

8. The bottom of patient demographics is where you can add any information on Medical History, Medications, Allergies, Social History, and Family History, to keep track of any important information about the patient. Simply click New or Edit to add or modify any information.
