With Arya’s Patient Portal, you can access medical records shared by your provider, including forms, prescriptions, results, and notes. You can also complete questionnaires and view your upcoming appointments.
Steps to set up an account for the Patient Portal:
Provide your healthcare provider with the email address you would like to use for your Arya Patient Portal.
Important: If multiple patient accounts share the same email address, a unique email alias must be used for each account through sub-addressing.
Your healthcare provider may create a sub-addressed email for your account. Be sure to confirm the exact email address they have assigned to you.
Use this unique email address when logging in to your patient profile.
2. When your healthcare provider shares a document with you for the first time, you will receive an email with instructions and a link to Create your Account.
3. Click on the Create Account button in the email to set up your account.
4. The Create Account page will open in your browser. To create your new account, you will need the following information:
Date of Birth
Health Card Number
A new password
5. Fill in your Date of Birth by clicking on the small calendar icon and selecting a date from the calendar.
6. Fill out your Health Card Number.
Important Note: If you are getting an error at this stage that tells you to contact your clinic, this means that the details you have entered (your date of birth and your health card number) do not match the ones your clinic has on file for you.
Please double-check that you are entering the correct details. If you are, please contact your clinic to make sure they have saved the correct details in your chart.
7. Create a unique password to access your account. (Password must contain at least 12 characters, including: one upper case letter, one number and one special character).
8. Re-type your password to verify it. The password must match the one you just entered.
Important: To access your Patient Portal, you will be required to create an account and set a password. For your convenience and continued access to your health information, we strongly recommend saving your password in a secure password manager. This will help ensure that you can easily sign in to your account in the future and reduce the risk of being locked out due to a forgotten password. Please keep your login credentials confidential and do not share them with others.
9. Select the check box at the end to accept the Patient Portal Terms and Conditions, Privacy Policy and Consent to use the Portal.
⚠️ You must agree to the Patient Portal Terms and Conditions, Privacy Policy and Consent to use the Portal in order to access the Portal. You can review each document by clicking on each of the links. The pages will open in a new tab in your browser.
10. Click on the Create Account button at the bottom of the page.
11. You will then be re-directed to the Confirmation page with a message that you have successfully created an account.
Now, you can access your account by selecting the Login button.
12. Enter your email address and password and click on the blue Login button to continue.
